Information you can send here
Service inquiries, caregiver applications, IHSS interest notes, satisfaction surveys, and grievances ask for contact details and the answers on those forms. Do not include medical details, a Medicaid ID, or a Social Security number. Applications do not ask for an SSN.
How those forms are handled
Submissions are emailed to the agency. They are not saved in a website database, and the site is written so the contents are not stored in application logs. If resume upload is configured, the file is placed in a private Vercel Blob store and a copy is attached to the email. If email is not configured, the form tells you to call or email us instead.
Forms that stay off this website
Intake, referrals, representative forms, releases, HIPAA acknowledgments, schedule changes, incident reports, and EVV or timesheet issues can contain health information, identifiers, or signatures. Those are hosted in a HIPAA-ready tool when the owner connects it. This website does not receive those submissions.
What we do not do
We do not sell personal information. This site does not load advertising or analytics trackers. If the owner turns on Cloudflare Turnstile, that check is used to block spam.
How long information is kept
Email and any private resume files are kept under the agency’s own retention practice. Counsel should set that period. People can call or email to ask about a submission they sent through the site.
Colorado privacy
Counsel should align this draft with the Colorado Privacy Act before it is adopted. The intended practice is that the agency does not sell personal data collected on this site.
